Permission Slips and Medical Forms

Posted on Apr 22 2012 - 11:28am

Hello Troop 5,

Starting with the May Bicycle trip to the Cape the Troop will no longer be collecting paper permission slips for each trip. The paper permission slip is being replaced by a combination of an online trip consent form along with a copy of the BSA Medical Record Form that we require to be on file with the Troop.
